Setup Patient Form

Patient Form is for prospective patients to complete when booking an appointment via Appointment Link.

You may set what type of information that you require from your patients. e.g address, date of birth. You can also include the 'Where do you hear about us question here'.

Example of Patient Form:

Steps:

1. At Appointment Settings menu, go to Patient Form
   
   
   
   
   
   
2. Name, IC/Passport No & Phone number are mandatory. Email & Notes field will be displayed, but is not required.
   
   
3. To add more field, scroll down, and choose Add field option
   
   
   
   
4. Choose input type:
   • Text
   • Selection input
   • Number only
   • Address
   • Text Area Field
     
     
5. Example:
   
   
   
6. Insert the placeholder. For this part, you may use an additional hint, description, or example of the information required for a particular field. These hints typically disappear when the user types in the field.
   
   
7. Click Save
